抄録 :Cough is one of the protective mechanisms of the respiratory system, and coughing can expel unwanted materials that enter the airway. In clinical settings, volitional coughing is, therefore, encouraged to patients with dysphagia, as a preventive method against silent aspiration. In the past, various research studies have been conducted to elucidate the mechanisms of airway protection. However, it is still not clear whether repeated coughing can influence the cough threshold of an individual. This study aimed at examining the influence of repeated coughing upon cough threshold of healthy young adults. Fourteen subjects (10 males and 4 females, mean age of 22±3.75 years) participated in the study. The subjects had no history of respiratory problems, asthma, and smoking, and no symptoms of a cold at the time of experiment. Also, in these subjects, no reflexive cough was elicited in response to inhalation of saline. The baseline cough threshold was determined in each subject by inhaling the nebulized citric acid solution, which was prepared in 5 levels of concentration. The concentration level that elicited more than 5 reflexive coughs was defined as the cough threshold of each subject. This procedure was conducted 3 times on separate days. During the third trial, cough threshold was measured after 20 repetitions of volitional coughing. In order to examine the relations between the cough threshold and respiratory functions, vital capacity, tidal volume, expiratory reserve volume, inspiratory reserve volume, forced expiratory volume in 1.0 second, and forced expiratory volume 1.0(sec) % were measured. There was no statistically significant difference between the cough threshold obtained in the normal condition and that obtained after repeated coughs. No strong correlation was found between cough threshold and respiratory functions. These results imply that repetitions of volitional coughs do not influence cough threshold, and cough threshold and respiratory functions are not strongly related.
