抄録 :This study was performed to investigate the influence of changes in posture during feeding on masticatory efficiency. In the clinical site, in patients with swallowing disorders, patients are taken a safe posture to direct training and feeding. Also, it is important for physical therapists to make patients with swallowing disorders the best posture for eating effectively. Subjects were 23 healthy adults (age,21.2±0.74 yr; mean ±SD). The subjects were asked to take four postures in random order: 1) reclining 90 degrees + head and neck facing straight forward (90-0 posture), (2) reclining 90 degrees + head and neck 30 degrees in the flexor direction (90-30 posture), (3) reclining 30 degrees + head and neck facing straight forward (30-0 posture), and (4) reclining 30 degrees + head and neck tilted 30 degrees in the flexor direction (30-30 posture). Gumi-jelly was given to the subjects for chewing. They were asked to chew it on the side that they normally chew and then to swallow it in one gulp. The activity of the masseter muscle was recorded by electromyography from the beginning of chewing to the end. The number of chewing strokes, total chewing time, chewing rhythm ( burst duration, burst interval, cycle time ) and coefficient of variation of chewing rhythm were compared with each posture. The number of chewing strokes was significantly greater in 30-30 posture than in 90-0 posture (p <0.05). The total chewing time was significantly longer in 30-0 or 30-30 posture than in 90-0 posture (p <0.05). Burst duration was longest in 30-0 posture (p <0.01 compared with 90-0 and 90-30 postures, p <0.05 compared with 30-30 posture ). Cycle time was significantly longer in 30-0 posture than in 90-0 posture (p <0.01). The coefficient of variation of burst duration was significantly greater in 30-0 posture than in 90-0 posture (p <0.05). From the results, it can be said that masticatory efficiency is the best in 90-0 posture. Therefore, it is suggested that it is better for you to set patient in 90-0 posture when there is only a dysfunction of mastication and is no swallowing disorderes.
