抄録 :Most of the rehabilitation programs of eating and swallowing functions aim at motor function recovery and there have been few cases in which sensory function recovery is aimed at. Furthermore, there has been very little investigation in which a quantitative evaluation of the age-related changes in oral somatic sense had been performed. In the present study, comparison of somatic sense of center of the dorsal lingual, which plays an important role in the bolus formation in order to swallow foods, was made between young people (aged between 18 and 23 years) and elderly people (aged between 67 and 78 years). 1) Measurement of tactile sense threshold. Tactile sense thresholds of the individual subjects were measured by stimulating the center of dorsal lingual using a Semmes Weinstein Sensory Tester. The threshold values of elderly people were significantly higher than those of young people in Welch test (two-tailed test). 2) Measurement of two-point discrimination thresholds. The measurement of two-point discrimination thresholds was performed at midline of the tongue in both horizontal (sideways) and sagittal (long way) directions. In the measurement performed both in a horizontal direction and in a sagittal direction, the thresholds of elderly people were significantly higher than those of young people in unpaired t-test (two-tailed test). Moreover, regardless of age, the thresholds measured in a horizontal direction were lower than those of sagittal direction. 3) Shape discrimination. Discrimination of wooden small pieces of two different shapes, one was circular with 1 mm thickness and 1.5 cm in diameter, and the other was square about 1.5 cm in a side; was tested. Center of the dorsal lingual of the subjects was pressed by the small pieces so that the entire surface of the piece is in contact with the tongue and with the power stronger than the tactile threshold. Two kinds of pieces were presented 10 times randomly and the subjects were asked to answer orally whether it is circular shape or square, then the ratio of the correct answers were calculated. The ratios of correct answer in elderly people were significantly higher than those of young people in unpaired t-test (two-tailed test). Taken together 3 different kinds of experiments shown above, somatic sense and shape discrimination ability decreased with aging. Sensory function of the tongue is considered to be decreased even in the healthy elderly people without marked eating and swallowing disorder, therefore, it is necessary to understand that the elderly people have disadvantages in eating and swallowing abilities and perform the evaluation and training in this area.
